Dabigatran versus Warfarin in Patients with Mechanical Heart Valves

The use of dabigatran in patients with mechanical heart valves was associated with increased rates of thromboembolic and bleeding complications, as compared with warfarin, thus showing no benefit and an excess risk. This was the outcome of phase 2 study indicate that at the doses tested, dabigatran was not as effective as warfarin for the prevention of thromboembolic complications in patients with mechanical heart valves and was associated with an increased risk of bleeding. These results might be explained by the relative inability of dabigatran to suppress activation of coagulation that occurs when blood is exposed to the artificial surfaces of the valve prosthesis. The use of dabigatran has no positive value and was associated with excess risk in patients with mechanical heart valves.

Among patients with TIA or minor stroke who can be treated within 24 hours after the onset of symptoms, the combination of clopidogrel and aspirin is superior to aspirin alone for reducing the risk of stroke in the first 90 days and does not increase the risk of hemorrhage. the double blinded study was done in 114 centers in China and results were published in New England Journal of Medicine. please click here to review abstract

WAKE-UP - a large European clinical trial to test whether current rtPA for stroke thrombolysis treatments can be used in patients who wake up with the symptoms of stroke. The first patients were enrolled in UK at the Southern General Hospital this week.
